BMV Transfer on Death Form

You can use this form to set up transfer on death for cars and other motor vehicles in Ohio. This will let you transfer your car to your loved ones without a will and avoid probate court. 

If you're married, your spouse can transfer an unlimited number of vehicles, worth up to $65,000, to themselves after you die using the BMV Surviving Spouse Affidavit without this TOD.

  • Take the form to a notary, sign it and have it stamped. 
  • Make a copy for yourself.
  • Submit the form at your county's Clerk of Courts Title Office. When you submit the form, they will reissue a title that includes your TOD. There will be a small fee for the new title.
