Financial Power of Attorney Revocation Form
Use this form to cancel an existing Financial Power of Attorney (FPOA). You have the right to end your FPOA at any time.
If you decide to end your FPOA, tell your agent. They must stop acting on your behalf once you tell them that you revoke the power of attorney.
If your Financial Power of Attorney gave your agent authority over real estate and was recorded in the County Recorder's Office, you need to file this form in the same County Recorder's Office.
- Attach a copy of the original Financial Power of Attorney form.
- Sign the form in front of a notary public.
- Make copies. After the form is signed, make several photocopies.
- Share the form. Send a copy of the revocation to your old agent and any financial institutions, like banks, that have a copy of the FPOA on file.
- If your Financial Power of Attorney gave your agent authority over real estate and was recorded in the County Recorder's Office, you need to file this form in the same County Recorder's Office.
